BRC Vacancy: Service Co-ordinator & Team Support Worker - Next Steps Assisted Discharge

The British Red Cross helps vulnerable people in crisis, whoever and
wherever they are.

 

Are you an energetic organiser with excellent communication and interpersonal skills? 

Can you co-ordinate and motivate a team of committed and enthusiastic volunteers?

Are you able to provide support and assistance to older people suffering from a health

crisis, assisting them to restore and maintain their independence?

 

Two positions have arisen within our newly created service 'Next Steps' Assisted Discharge Programme in London.

                  

Service Co-ordinator 'Next Steps' Assisted Discharged

Salary:  £22,933 per annum included ILW 

12 months fixed term contract - 35 hours per week          

Location: Chelsea & Westminster Hospital

Team Support Worker 'Next Steps' Assisted Discharge

Salary: £17,214 pro rata per annum included ILW

12 months fixed term contract - 20 hours per week                                 

Location: Chelsea & Westminster Hospital

Ref: SE12345

   

We require two enthusiastic and motivated members of staff to ensure the success of a new and rewarding project providing support and assistance to vulnerable, older people bridging the transition from hospital discharge to home. The team will be working with and supporting a team of dedicated volunteers, hospital discharge teams, and local agencies to reduce the unscheduled re-admission of service users.

 

For both positions, experience of working with older people and vulnerable groups would be an advantage as would project development experience for the Service Coordinator role. The Team Support Worker should have access to a car. The successful candidates will be proactive in raising awareness of the service and publicising the service to appropriate groups. They will also be able to respond to out-of-hours situations
